Strategic Context

To meet the requirements of the Strategic Vision ‘Towards the Best Together’ requires a significant step change in the ability of staff to deliver palliative and end of life care.  About 70% of people in the east of England express a wish to die in a place of their own choice. This is not being realised at present for a number of complex interrelated reasons, one of these being a lack of specific skills amongst practitioners.

Some patients are also being admitted inappropriately to acute hospital services and one of the outcomes anticipated from this training is to reduce inappropriate admissions.

The drive to improve quality, defined as safety, effectiveness and patient experience, requires education and training investments that enable our staff to think differently and behave in new ways to deliver this demanding agenda. 

Aim

Proposals are sought from Primary Care Trusts within the East of England to work with the NHS East of England and participate in three end of life care pilot studies which will focus on the use of Gold Standard Framework (or GSF equivalent) tools in a ‘whole systems’ approach; to research nursing homes and primary care providers. An independent evaluation will commissioned to examine the impact of the three approaches.
