I love it when GPs are ahead of the curve. If we want FTs to embrace paperless records then we find a system that makes it impossible for them to bill commissioners without an electronic trail. Add in a few requirement like electronic discharges and prescribing and economics will ensure things change. We GPs could not get our QOF money without functioning IT. The unintended benefit is that it is good for patients

I am interested in the quote at 8:40 about where people wish to die. Does anyone have any data about the wishes of those living in care homes? I know my CCG is looking into preferred place of death and being very proactive. As everyone has said it requires the family, the home, the patient and the GPs all to be singing from the same hymn sheet

I probably should not say anything but I will. warfarin is a problem drug, But the idea that the newer versions are any better is purely an industry led myth. They are untried and untested in the real world and could easily lead lead to more problems than they are meant to solve. this quote ' Mr Ridge gave the example of Warfarin, an anticoagulant drug which is a significant cause of medicines related admissions to hospital. He said use of new oral anticoagulants could reduce these admissions and lead to a reform of anticoagulation services' suggests too close an involvement with big pharma than I think is appropriate. Who does the 'chief pharmacist' answer to?

Comment on: Revalidation cannot be left to GMC alone
Andrew Craig writes clearly and convincingly for a strong form of assessment. That is exactly what Revalidation will not provide. Appraisal is all about knowing the latest guidelines and avoiding too many complaints. Five complete appraisals equals guaranteed revalidation. There is not a single element of assessment of ability in it. Yes there is an occaisonal feedback form for colleagues and patients but that will just ask if you are a nice guy/girl, not whether those who know would let you treat their family. I think it highly unlikely that CCGs have anywhere near enough maturity or resource to look to performance management of clinicians in the near future. I don't know the right answer to rooting out poorly performing Doctors, but I do know that revalidation in its current form is certainly not the answer.

Comment on: HSJ Efficiency Award winners announced
Well done to all the winners. I had a very small peripheral part to play in the Sussex project and know how hard the managers worked to get this completed, so i appreciate how much work went in from all the teams. I must remember to point our local politicians to this next time I hear them bad-mouthing NHS management.
