    Even after the big pay cut documented above on average earning more than the PM. Hopefully there is a good balance of 'be - ers and do - ers on this quango (correction NDPB). Presumably there will be regional executive boards and local directors with management teams. Is it conceivable that among the forest of execs there will be room for the occasion GP or primary care practitioner.

    So the NCB is the NHS politburo and its local outposts will be telling GP's and CCG's how they deliver a GP lead NHS. Should be a rich source of public conflict and outrage.

    Or maybe they could outsource their PP's to private hospitals under a partnership agreement thus lessening any conflicts of interest and confusion with the NHS facility. It is inevitable that FT's will come to own private health facilities before 2015.

    I would have been interested to read Andy's take on whether the current nationwide 're-configuration' militates against the realisation of the undoubted potential for productivity and efficiency gains across the NHS in the QIPP time-scale. Is quality the only consideration for centralisation? As far as I am aware the top of the quality scale does not yet cover resurrection when a patient is beyond treatment after an hour in the ambulance. It is outcomes that really matter.

    It is not clear to me that Mr Palmer will provide the 'grit' in Monitor's competition oyster. Naivety is the word I would use for the DH presentation. What needs regulating is the external procurement of goods and services including when competition does not come into it. The public sector is poor at managing both soft and hard contracts because it tends to underestimate the resources needed and in some cases ill - define responsibilities. FT's will test CCG's ability to manage suppliers in a quasi commercial environment.
