Comment on: Staff should be protected from 'never events'
As ever, Rajan makes a really important point. The key to reducing error in all healthcare settings is for leaders to be aware of what's going on at the front line of service. This is impossible without listening to what staff, who are experts in service delivery, have to say. That requires us to think through the enormous courage it takes to raise a concern, and to respect and protect that disclosure. Failure to do so is a never event.
