Trafford PCT is inviting expressions of interest from suitable qualified organisations to tender for the provision of the community based Chronic Obstructive Pulmonary Disease Service in adults.
The contract will be for one year to develop a model of care that supports patients in primary care. The overall aim of the service is to offer safe and effective community support, reducing the need for hospital admission where appropriate.
