PERFORMANCE: Wye Valley Trust received a mortality outlier alert from the Care Quality Commission in November.

Notes from 13 cases, which were all in the fluid and electolyte disorders category, have been reviewed by the medical director and senior coder with input from other clinicans.

A report to the West Mercia primary care trust cluster board, which includes NHS Herefordshire, said an action plan has been developed by the trust and cluster medical director.

It stated: “Wye Valley Trust has developed a policy to review the case notes of every mortality case from now onwards. These will be reviewed initially by clinicians with follow up review by the medical director and external review where required.”
