University Hospitals Coventry and Warwickshire Trust: Patient safety and risk team

With a Care Quality Commission report identifying that University Hospitals Coventry and Warwickshire Trust had low incident reporting rates – and that staff who did make reports sometimes felt unsupported and patients sometimes uninvolved – members of the patient safety and risk team set about addressing the issue.

They instituted a raft of measures over the course of 2017, including a redesign of the incident reporting and investigation form; the introduction of electronic safety dashboards for all areas; the creation of the patient safety response team, through which the trust’s most senior clinicians attend any area with a report of serious harm to ensure staff feel supported; and improvement of the duty of candour processes, not least through the development of a patient information leaflet.

Since the changes, the incident reporting rate at the organisation has increased from 31 to 44 incidents per thousand bed days – in the top 25 per cent nationally. There have also been improvements in staff survey responses on feeling secure when raising concerns.
