PERFORMANCE: A fax server failure has led to a backlog in cancer waits in Berkshire.
Primary care trust cluster board papers said that in month four of 2012-13, performance against the two week wait standard dropped, with 91.1 per cent of patients seen within two weeks following an urgent GP referral.
The target was 93 per cent, leaving the area with a red year to date rating . The papers said: “In July a technical problem with the fax server occurred. This was the first failure of its kind in 10 years.
“Operational procedures for checking the fax server daily are in place. However, these were not followed correctly by booking centre staff, leading to a lack of awareness of the problem.
“When the problem was identified, a number of referrals were received in a batch with a maximum delay of seven days.”
Operating procedures were also not followed at Heatherwood and Wexham Park’s Parapet Breast Unit. Administrative staff had been recording referral dates incorrectly.
