PERFORMANCE: Berkshire West primary care trust has reported an increase in serious untoward incidents (SUIs), including a rise in the number of recorded pressure ulcers during 2010-11.

The trust said it had identified the issue as a “main area of concern”, despite the fact it had a already had a detailed action plan in place.

In 2010-11 there were 79 grade 4 pressure ulcers – the most severe level – compared with 53 the year before, and 22 other SUIs, up from 14 in 2009-10.

The trust also recorded 63 grade 3 pressure ulcers. Trusts were required to record these as SUIs for the first time in 2010-11.
