PERFORMANCE: Delayed transfers of care at Birmingham Community Healthcare Trust are running at more than twice the NHS West Midlands target.
The percentage of delayed transfers of care was 7.1 per cent at month four and continues to exceed the SHA target of 3.5 per cent.
The reasons for the delays are largely due to delays in assessment and patients waiting for either a care home placement or a package of care having been assessed. This is due to the lack of a social work co-ordinator for the community hospitals.
There has also been an increase in the number of homes asking for additional “top-up” payments and as a result patients and/or families have been refusing to accept places in these homes; this accounts for the increase in the number of days delayed by patients or families exercising choice.
Additional delays have been caused by one patient requiring a culturally appropriate, dementia registered nursing home and some seasonal issues have
been experienced with social workers taking annual leave in the summer and cover being unavailable due to the small numbers in the team.
