PERFORMANCE: Nearly one in five patients admitted to Birmingham Community Healthcare Trust are experiencing delayed discharges.
Latest board papers show the number and percentage of delayed transfers of care have decreased slightly but remain above target. Nearly a fifth of patients are being delayed, despite a target to bring numbers down to below 2009-10 levels, or 11.6 per cent of admitted patients. At month 11 of 2010-11 delayed discharges were above the total number in the previous financial year.
The reason for these delays is mainly due to the lack of a social work co-ordinator for the community hospitals which has resulted in delays in assessment and patients waiting for either a care home placement or a package of care having been assessed.
Meetings have taken place with some improvements made to social care support in some areas and further actions proposed.
