PERFORMANCE: NHS Bournemouth and Poole is concerned about its performance on transfers of care.

A report presented to the board suggests the high proportion of the population who are elderly increases pressure on the PCT which is assessed on the number of acute and non acute delayed transfers of care as proportion of the registered population. For the period ending September 2010 the PCT was in the bottom quartile nationally. The PCT has agreed a number of actions to address the problem including weekly patient level monitoring; hospital focus on improving discharge planning and communication; monthly case reviews of people delayed for 30 days or more; funding out of hospital agreements and transitional beds; targeted work with care homes.
