PERFORMANCE: Delayed transfers of care could become a major issue in Cambridgeshire this winter, the NHS Cambridgeshire board was told.
The latest data showed that Cambridge University Hospitals Foundation Trust had 29 delayed discharges against a target of 11. Issues accessing home-based care and putting care packages in place affected discharge from both the acute trust and interim care beds.
“This has been identified to the SHA as a major concern for winter 2011,” a report said.
New care providers have been identified and some patients could be given “live in” care at their own homes instead of going to interim or residential care.
