The Department of Health has issued a report on improving support patient safety in the NHS.

Safety First - a report for patients, clinicians and healthcare managerssays the National Patient Safety Agency should be 'refocused' on collecting and analysing information through its national reporting and learning system, and that this should be easier for staff to use.

It also puts a stronger emphasis on local measures to 'embed' safety in organisations. Its recommendations include: the establishment of a national forum to bring together key agencies; patient safety teams to support frontline staff; a national campaign on patient safety issues; and increased involvement for families and patients.
