PERFORMANCE: The Blackburn with Darwen coroner has issued East Lancashire Hospitals Trust with two Rule 43 reports, board papers state.

Coroners can issue Rule 43 reports to an organisation if evidence taken during the course of an inquest gives them cause for concern that action should be taken to prevent future deaths.

A report to East Lancashire’s 31 July board meeting said that the reports related to “changes in procedures”. It added that for “reasons of patient confidentiality” the cases would be considered in the private section of the meeting, but said that “arrangements have been put in place to meet the issues raised and to ensure the full response is provided to the Coroner within the required timescales”.
