PERFORMANCE: 2gether Foundation Trust hit the target for following up 95 per cent of patients within seven days of discharge from mental health inpatient units.

However the trust fell short of a tighter self-imposed target to contact patients within 48 hours.

In Gloucestershire the trust saw 91 per cent of patients within 48 hours. In Herfordshire 78 per cent of patients were seen within the same timeframe.
