The shift from hospital to community is front and centre of the government’s 10-Year Health Plan for England. Enabled by improvements to digital infrastructure, the plan is to reduce fragmentation, provide a more connected experience for patients and improve health outcomes.

But looking beyond metrics such as reduced hospital stays, lower mortality, and lower rates of disease, some analysts predict that connected care has further knock-on effects for society. It can reduce the disruption that patients often experience in managing complex conditions and allow them to remain at home, at school, and at work for longer. Families experience less disruption, and underserved communities have more equitable access to services.

This HSJ  webinar, in association with Sciensus, brings together experts from the NHS, think tanks, policy and industry to look at the data behind health inequalities and the impact that technology-facilitated, community-centred care can have. The panel will also explore how NHS organisations are rethinking pathways to bring care closer to home and break down barriers to social mobility.

The Rt Hon Justine Greening is a leading advocate for social mobility and equal access to opportunity, with a career spanning senior roles in government, business and the social impact sector.

Justine served as MP for Putney, Roehampton and Southfields for 14 years, including more than six years in the UK Cabinet under two prime ministers. She is the founder of The Purpose Coalition, which brings together business leaders, public sector leaders, educators and policymakers to take a collective, practical approach to improving social mobility across the UK.

Jonathan has a long and distinguished career within UK healthcare, with expertise in health system organisation, accreditation, governance and regulation. Dedicating his career to nursing, he is the inaugural president of the Nursing and Midwifery Council and a fellow of the American Academy of Nursing. Alongside helping Sciensus deliver excellent service for patients, Jonathan serves as president and chairman of the Council at the European Society for Person-Centred Healthcare and is also a non-executive director of an NHS Trust.
