FINANCE: Leeds Health and Social Care Transformation Board - bringing together the health and social care bodies in the city - has begun leading service change in several areas.

The Leeds Health and Social Care transformation programme is a “city-wide agreement between health and social care partners to work together to improve quality and increase innovation and productivity, driving forward the transformation of local services”.

A PCT board paper says: “Changes are now being implemented in the areas of Type 2 diabetes, home oxygen services and the risk stratification of patients with complex conditions who are most at risk of admission to hospital.

“These will see improved efficiencies and better use of resources alongside improvements to the care received by local people. The programme board continues to oversee the development of cases for change in urgent care services and to improve support for older people with long-term conditions.

“In all instances, any proposed changes will be developed and delivered in accordance with the existing arrangements established with the scrutiny board (health) in Leeds.”
