Winner: Barnsley CCG

RightCare Barnsley is a care coordination centre which helps ensure patients are cared for as close to home as possible. A collaboration between the clinical commissioning group and the local provider trusts, the centre is staffed by hospital and community nurses and acts as a single point of referral to all health and social care services. This includes referrals for support following hospital discharge.

Staff have access to both hospital and community clinical systems, as well as the ability to input details on both. This ensures seamless transfers of care for patients, and less duplication.

Between February 2015 and October 2015, 1,811 referrals were handled by the centre. Evidence suggests hospital admissions were avoided in 34 per cent of cases, and that length of stay is shorter when an admission is required. The average call is answered within eight seconds, and handled within 16 minutes.
