STRUCTURE: A new hospice at home service has been commissioned in west Norfolk and should be operational from September.
It will be provided by Norfolk Community Health and Care NHS Trust in partnership with Norfolk Hospice – Tapping House and Marie Curie Cancer Care.
Two additional palliative care nurses, recruited by Norfolk Hospice will improve training in palliative care among care home and healthcare staff. Norfolk Hospice and Marie Caurie Care will also expand other staff numbers.
Referrals to the service will come from GPs, acute hospital staff, and community-based NHS teams, as well as by care homes and other residential settings.
The trust’s community teams will continue to lead on the day-to-day provision of nursing, therapy.
The trust will be expanding its seven day a week coordination team to organise care through a central hub.
