SERVICES: A service aimed at keeping patients out of hospital is now being run by Norfolk Community health and Care Trust.

The out of hours community nursing scheme was previously run by the ambulance service. The trust hopes to provider a “responsive, effortless and seamless” nursing service to patients with the out of hours service operating between 7pm and 8am.

The service is among three new services which will be provided following discussions with commissioners. These include a ‘night sitting’ service and a dedicated “admissions avoidance” nurse per practice, who will support proactive management of vulnerable patients.
