PERFORMANCE: A patient death and two cases where patients suffered severe harm are being investigated as serious untoward incidents by North Cumbria University Hospitals Trust.

A report to the board said that 60 other incidents, also reported in April, thought to have a moderate impact were being reviewed and families informed.

The trust also had five grade three and three grade four pressure ulcers which developed after admission: these may be declared as SUIs following investigation of whether they were avoidable.

The trust had 44 SUIs declared in 2012-1, five of which were never events.
