COMMERCIAL: Pennine Care Foundation Trust has entered into a partnership with care home company Meridian Healthcare Limited to provide post-discharge care in Tameside and Glossop.
The service will be provided from a new unit named Grange View, at Meridian’s Hyde Nursing Home, Tameside, the company said in a statement.
“It provides care to adults who are well enough to be discharged from hospital but need additional care before they can return home,” the statement added. “This includes patients from Tameside General Hospital, as well as neighbouring hospital sites.
“The clinical team is made up of nurses and therapists who will help patients to recover and regain their independence following a hospital stay. This will include patients who have a long term condition, those who need help with dressings and wound care and people with mobility problems.”
Pennine Care operations and nursing director Michael McCourt said: “Grange View will help to ensure patients don’t have to stay in hospital for longer than is needed because they can be cared for safely in the community, which aids recovery and improves the patient experience.”
