Atrial Fibrillation Virtual Ward

The project established a novel telemedicine-based model of care for acute patients with atrial fibrillation and rapid ventricular response - the AF virtual ward. People presenting acutely with AF or atrial flutter to the hospital were onboarded to the virtual ward and received holistic remote care at home. This was achieved through a multidisciplinary team; remote technology and digital equipment; smartphone app; online platform; pharmacy and actively involving the patients in their care plans. This reduces the huge AF-associated burden on healthcare without compromising on patients’ care or safety. The judges found this to be an impressive new intervention in an area of high prevalence, led by an engaged and enthusiastic team.
