Waiting Well With Suspected Autism Pathway 

Stort Valley and Villages has a pre-existing children and young people’s social prescribing service which has helped more than 1,500 people between 11 and 25 access support for physical and mental health issues that present via one of the five practices in the primary care network. The service has been hugely appreciated by many young people locally and has been expanded to include a neurodiversity pathway and a young homeless outreach service. The two new pathways have sought to reduce health inequalities and encourage young people who may struggle to access support find a way through the system. The judges found this to be a novel approach problem identification and management, that involved clear patient advocacy and multidisciplinary approach. This was a compassionate and innovative response in an area where it had evident impact.
