PERFORMANCE: The trust had four overdue patient safety alerts in September, according to latest central alerting system data.

Data extracted from the system on 20 September show that the Walton Centre had yet to complete actions in relation to the following alerts:

·        Preventing fatalities from medication loading doses

·        Safer spinal (intrathecal), epidural and regional devices Part A: update

·        Minimising risks of mismatching spinal, epidural and regional devices with incompatible connectors

·        The adult patient’s passport to safer use of insulin

The deadlines for completion of the alerts ranged from November 2011 to April 2012. The centre had the joint-highest number of outstanding overdue alerts in the North West in September, alongside Aintree University Hospitals Foundation Trust, the data show.
