The Asthma and Wheeze Project is overseen by the integrated borough-based partnership “Tower Hamlets Together” and sought to significantly reduce non-elective admissions of children and young people with asthma and wheeze to the Royal London Hospital. As a system the project mapped the primary and secondary drivers for admissions and agreed the need to commission an intervention that provided both a clinical and non-clinical response. From the interventions delivered, which included community and education based consultations as well as professional development, it was able to reduce non-elective admissions against our baseline and improve clinical metrics as well as self-reported well-being.
