PERFORMANCE: Barnet and Chase Farm Hospitals are investigating a series of ‘black breaches’ in which the time from an ambulances arrival to the patient being formally handed over to the trust was longer than 60 minutes.
The trust reported two in July and three in June, taking the year-to-date total to six.
A report to the board said: “An investigation is currently underway with a root-cause analysis and action plan report to follow. Following root cause analysis of the London Ambulance Service breaches in June 2012 the following key issues were identified: Lack of inpatient bed capacity. Poor inpatient discharge performance further compounding inpatient bed capacity issues. Subsequent impact in reducing A&E bed capacity. Lack of timely escalation.”
