The Department of Health has published a document setting out the new role for community matrons in case management. It follows health secretary John Reid's pledge to introduce 3,000 community matrons by 2008.
The booklet, aimed at workforce planners as well as practitioners, sets out an eight-point plan which health economies should follow to establish case-management systems.
Steps include identifying the population 'at risk' of admissions; defining the role of case management;
involving users and stakeholders in planning and service redesign; redesigning and skilling up the workforce; and identifying and preparing supervisors and mentors.
