The Department of Health is set to unveil a plan for chronic-disease management in September which will detail how the NHS at both strategic health authority and primary care trust level should approach implementation.Department of Health director of primary care Gary Belfield said that the plan would address ways of reducing the numbers of patients who most frequently use the NHS in order to 'release some acute capacity'.Mr Belfield said that the plan would also list private healthcare companies which specialised in chronic-disease management along the Evercare and Kaiser Permanante model to which PCTs could look for ideas on best practice.
