Recent articles in HSJ on personal medical services pilots have been interesting and timely. Both these articles and the NHS plan indicate that primary care groups and primary care trusts might hold and manage the contracts with PMS practices.

Surely this could create serious conflicts of interest and accountability? When the chairs of PCG boards or PCT executive committees are also partners in contracted PMS practices there are bound to be difficulties.

PCGs and PCTs have neither the management capacity nor the structural balance to cope readily with such problems.
