The programme is doing this by supporting local health communities with delivery of service transformation.

There are 135 local health communities across England, each of which brings together primary care, acute care, mental health and ambulance services. The aim is to provide seamless care services to those that need it.

The Demonstrators programme is working with 16 local health communities, who will each undertake benefits-led, integrated change programmes to deliver tangible, sustainable improvements in health and social care.
