Maidstone and Tunbridge Wells Trust is conducting an internal investigation after two biopsy patients’ records were mixed up resulting in the wrong treatment for each.

Medway Foundation Trust’s November board papers state: “The case of two patients whose biopsy reports were mixed up was noted. Both received the other’s treatments as a result. The error was not the trust’s [Medway’s] and once realised the response was rapid; Trust consultants arranged for each patient to be seen quickly. Maidstone and Tunbridge Wells Trust are now conducting root cause analysis with the trust’s assistance.”

A spokeswoman for Maidstone and Tunbridge Wells Trust said: “We are continuing to investigate this incident so are unable to comment at this stage.  However, as a trust, we have put measures in place to ensure this cannot happen again.  Our top priority is, and will always be, our patient’s safety and their wellbeing.”
