The East Leicestershire and Rutland Clinical Commissioning Group is asking for views on elective care services such as out-patient appointments, minor injury units and minor surgery.
The survey opened in early April and asks for views on where services are located and how often they are used.
It closes on 13 May.
Dave Briggs, chairman of the group, said: “Patient opinion is very important to us. We want people’s views to shape our plans for these local services from the very start so we can ensure they meet the needs of the community.”
