PERFORMANCE: The primary care trust’s rate of emergency readmissions within 30 days of discharge was 5.9 per cent, its latest performance report shows, against a target of less than 5 per cent.

The NHS Merseyside PCT cluster report noted that “although the overall measure of admissions avoidance is currently above plan, the actual activity has decreased by 5 per cent (30 per cent of emergency admissions are readmissions)”.

It added that “further inroads into reducing this level of activity” would be made with the introduction of a newly commissioned COPD service.
