PERFORMANCE: The South Central Ambulance NHS Trust has reported a series of serious untoward incidents involving equipment failure, including ambulance doors not opening.

A list of SUIs under review included one “failure of tail lift with critically ill patient” in Buckinghamshire in August, another “failure of tail lift and inability to open back doors of ambulance with critically ill patient” in North Hampshire during the same month.

There was also a “failure of back doors to open” in Oxford in September, and two non-specific incidents involving the “failure of clinical equipment”.

The list also included two road traffic collisions and the “failure of the Computer Aided despatch system”.
