East of England Ambulance Service Trust and the police have set up a joint investigation after a patient was found dead following the trust being unable to access a property.  

The trust’s quality report said: “The trust could not gain access to a property and the patient was found dead once the police arrived and accessed the property.

“This will be a joint investigation with the Police Force. This incident will be reported in the September 2013 figures.

The trust reported three serious incidents in July and two in August, according to report.

It said: “One [of the serious incidents in July] related to a loss of confidential information, one was relating to a patient assessment issue where the patient died over two weeks later and the third was related to a delay to a patient in cardiac arrest who also died.”
