After all these events the question is always ‘how could this happen’ I believe that one of the reasons is that we lack the imagination to make the leap to believing it could be true because subconsciously we think that other professionals think and behave as we do.


And perhaps worth remembering that when I worked in what was then geriatrics in the eighties there were no crash calls for anyone over 73 a situation unimaginable today. Retrospect is a perilous tool.
