STRUCTURE: South Warwickshire Foundation Trust has begun “scoping work” on two new transformation projects, both involving working with “strategic partners”.

The trust is exploring “integrated patient access”, which is looking at developing an easier way to access a number of services across acute and community settings through a single point of access.

This work is being taken forward in partnership with Serco.

Board papers for February reveal a strategic paper was under development for the delivery of integrated care for people with long term conditions. The project is also planned to involve the use of a strategic partner.

“It is anticipated that this will be a three-year programme of work led by primary care but working jointly with health, social care, voluntary and private sector organisations.”

Also up for discussion was an “assess to admit” project to ensure that only patients who require acute hospital care will be admitted, with alternative pathways in place for those who do not.
