Putting Communities At The Heart Of Decision Making Through System Integration And Learning

The population of Rochdale is one of the most deprived places in the country and organisers of this entry have developed a mature, integrated health, care and wider determinants system which is delivering improved outcomes. “We are recognised for this work and were asked to share on the global stage at the International Conference for Integrated Care in Antwerp.”

The integration has seen system-wide events bringing together partners from across the local system with patients and their carers to share learning from incidents. The panel felt a real sense of integrated partnership working here, with teams joining up to create a new ethos. “This is a true example of how places can learn and work together, and the reasons and ways that integrated care can make a difference,” said judges.
