PERFORMANCE: The trust is looking at how seven service users came to be discharged without Care Programme Approach plans confirmed.
Performance papers for WLMHT said each case was being checked to see if there had been data entry errors on the trust’s records system.
It said: “Seven-day follow-up after discharge [is] also below target at 92 per cent (target 95 per cent). This is most likely a recording issue and the board will be updated next month.”
The document reported that across the three local service directorates the number of “enhanced patients” - those with complex care needs - who hadn’t had a six-month CPA review fell to only five per cent in December, rising to seven per cent in January.
Another quality measure, patients readmitted within 28 days, saw the trust remain within its 10 per cent target, at nine per cent for January, December and November.
