H4All: High Intensity User Service

The High Intensity User service was launched to reduce the unnecessary attendances and admittances to Hillingdon A&E of the top 50 patients, which incurred an annual cost of £7m.

The team deliver a holistic model of support that utilises health coaching, integrative counselling and social prescribing for these patients, who do not fit into traditional systems of support, due to their acute conditions, chaotic lifestyles and the wider determinates of health that impact their wellbeing and drive their overuse of emergency services. The service is a partnership with the A&E, LAS, police, housing, substance misuse and mental health services.
