NHS Redbridge plans to target the 20 biggest users of A&E
FINANCE: The primary care trust hopes to save £100,000 in the first quarter of 2011-12 with the scheme.
The district nursing and long-term conditions teams will then design “individualised pathways” for each patient which they believe will reduce emergency admissions by two per service user.
The PCT will also be the site for a Health Foundation-sponsored project which will see 10 practices provided with intensive support to enable the implementation of NICE COPD guidelines.
This will see: “Both patients and their GPs receive individualised reports on the quality of their COPD care focusing on interventions known to reduce admissions.
“GPs will be able to identify gaps in care at individual patient level. Patients will use the information to request better care. This will lead to improved quality of care as there will be less need for emergency admission. Detailed data of the quality improvements and the financial implications will be available at individual patient level.”
