You've missed the biggest one: not invented here syndrome.

The "inclusive" approach the NHS always takes encourages people to develop their own solutions. Heaven forbid you try and make people do it differently without them figuring out that way themselves (and probably coming up with a different name for it).

I remember banging my head against a brick wall numerous times when I had two frontline teams (result of a merger) both insistent that they weren't going to standardise because their way was best.
