PERFORMANCE: A patient had the wrong lens fitted during eye surgery at the Sandwell and West Birmingham Hospitals NHS Trust.
A quality report by the trust revealed the incident, classed as a “never event”, happened in the ophthalmology department in March.
At the end of the operation, it was discovered the wrong lens had been implanted.
The patient was told about what had happened and agreed to further surgery to implant the correct lens.
The quality report by the trust said: “The never event investigation process has been instigated and a review meeting took place on April 3. The findings and lessons learned from this incident will be shared widely.”
