More cash for health is welcome (news, pages 4-5 and comment, page 21, 30 March). But where it goes, and how it gets there, is crucial for it to work.

Delegating money through tiers of authorities - nationally and regionally, ending up with unevenly developed primary care groups and trusts, not always aligned with their local councils - means the good motivation may get lost in the mechanics.

And improving health for us all requires action by many government departments (especially for children, as highlighted by UNICEF): we need to shift resources from those people and communities with most wealth and best health towards those with least and worst.

Developing a target for tackling health inequalities, joined up across government, must go hand in hand with delivering the cash.
