The Information Centre for health and social care is at the heart of UK-wide initiative to develop information to support community care.
A recent meeting of the Adult Care Information Network looked at how information can improve the transition of service users from hospital to being supported to live in their own homes. Delegates at the forefront of information development from around England, Scotland and Wales used case studies to see how information can make an impact at each step of a client's journey. Knowing what each client wants is seen as key to measuring whether the care had a successful outcome.
The group is now sharing innovative thinking from around the UK on how information can be used to improve services. It is also considering how work in each of the four countries can be aligned, demonstrating best practice in government collaboration across the UK.
