The Scottish Executive has launched a consultation into patient safety to encourage a move from a blame culture to a 'reporting culture' and 'safety culture'among NHS staff.The results of the consultation, which closes at the end of February, will be taken forward by NHS Quality Improvement Scotland, the successor body to the Clinical Standards Board and other quality organisations.Scotland will not, however, have its own national patient safety agency but will feed information to the English body.
