James Titcombe
James Titcombe is the father of Joshua, who died as a result of preventable errors during his care in 2008. James is chief executive of the charity Patient Safety Watch, patient safety ambassador for University Hospitals of Morecambe Bay Foundation Trust, a campaigner for safer healthcare, an ambassador for the charity Baby Lifeline and a freelance patient safety consultant.
