Blackpool Teaching Hospitals FT: Giving Back the Gift of Time - Improving the Last 1,000 Days of Life

Patient time is the most important currency in health and social care. Many people who get stuck in the healthcare system are in the last 1,000 days of their life, so they are the very people who do not have time to waste. Blackpool Teaching Hospitals Foundation Trust wanted to work with local partners to improve the last 1,000 days of life for their frail and elderly population. Working in partnership with care homes, residents have been “given back the gift of time” to spend in the place they love, with the people they love.

The judges said it showcased good evidence of cross organisational and place-based partnership working. The way they evidenced and tracked their improvement was impressive and their patient stories were very powerful.
